• Photo: Face should occupy at least 75% of photograph, good quality passport size color photo with white background.
• Signature: Draw rectangular box (2 cm × 7 cm) on A4 size paper and sign with black/blue pen in running hand.
• Thumb Impression: Draw rectangular box (3 cm × 5 cm) on A4 size paper and mark left thumb impression.
